You must complete and submit the survey on the same day your subscription is processed. After processing your subscription, look for the survey link in your order confirmation email and complete the survey that same day.

Without an LMN, products like fitness equipment, supplements, or sleep technology are considered general wellness purchases and do not qualify under IRS rules, regardless of their health benefits.

- Your HSA/FSA administrator has final authority. Truemed’s LMN supports your claim, but your plan administrator makes the final determination on reimbursement.

Letters of Medical Necessity are typically issued within 24–48 hours. In some cases, additional time may be needed for the TrueMed provider team to review your qualification survey.

HSAs and FSAs are typically offered through employers and selected during open enrollment. Check your employer's benefits portal or contact HR to see whether you already have an HSA or FSA.
You can also open an HSA independently through many banks and HSA providers, as long as you're enrolled in a high-deductible health plan (HDHP). FSAs are only available through an employer.

Being in a supported category does not guarantee eligibility. Two customers purchasing the same product may receive different outcomes based on their individual health profiles. Qualification is always assessed on a case-by-case basis.

- A customer with a diagnosed sleep disorder may qualify for a mattress or sleep tracking device.
- A customer managing cardiovascular disease or metabolic conditions may qualify for fitness equipment or a gym membership.
- A customer with a documented nutritional deficiency or gastrointestinal condition may qualify for specific supplements.
- A customer managing chronic pain or musculoskeletal issues may qualify for recovery tools like red light therapy or cold plunge equipment.
General health goals, like wanting to lose weight, get stronger, or sleep better without a specific underlying condition, do not meet the IRS threshold on their own. There needs to be a medical reason that goes beyond general wellness.
This is intentional. The IRS requires that HSA/FSA spending be tied to medical necessity, and Truemed’s qualification process is designed to make sure purchases meet that standard.
When you complete a purchase through a Truemed partner brand, you will be asked to fill out a brief health intake form, sometimes called a qualification survey. This form collects information about your current health, any relevant diagnoses or conditions, and why you are purchasing the product.

The reason Truemed requests this information is so our provider team can evaluate whether there is a medically necessary reason for your purchase of a specific product or service. Being able to identify a reason above the baseline of “general health” is a requirement for customers who want to use their HSA/FSA funds toward certain products.
Here is what happens after you submit the intake:
- An independent, licensed clinician reviews your health information.
- The clinician assesses whether your purchase qualifies as medically necessary given your specific health context.
- If your purchase qualifies, you receive a Letter of Medical Necessity (LMN) that supports your payment or reimbursement.
- If your purchase does not qualify, you will be notified.
Not all purchases are guaranteed to qualify. Eligibility is determined on a case-by-case basis. Truemed does not guarantee any specific outcome.

When your LMN expires, the documentation is no longer valid for new purchases. To renew, reach out to Truemed support at support@truemed.com. The team will send you a link to retake the clinical intake survey, and a clinician will review your updated information. If medical necessity is still supported, a new LMN will be issued.
For one-time purchases (like a piece of fitness equipment), renewal isn’t necessary unless you plan to make a similar purchase after your LMN has lapsed. For ongoing purchases, such as supplements, you’ll need to renew annually to keep your documentation current.
Each LMN is specific to the merchant and products it was issued for. Technically, an LMN issued for one business cannot be transferred or applied to a different merchant’s products or services.
This is a critical step in ensuring that your purchases are compliant with IRS guidelines. The IRS requires that an LMN directly relates to the product or service being purchased. If you plan to use HSA or FSA funds at multiple merchants, you’ll need a separate LMN for each one. Troubleshooting Your Qualification Survey
The qualification survey is how Truemed’s licensed clinicians verify that your purchase is medically appropriate for your health needs. Most customers complete it in just a few minutes, but occasionally something goes wrong. This article covers the most common issues and how to resolve them.
When you order multiple products that fall under different health categories, each one needs its own eligibility check. To use HSA or FSA funds, a purchase must be medically necessary for a specific health condition. A product that addresses sleep issues draws on different diagnostic criteria than one addressing joint pain or cardiovascular fitness, even if they come from the same cart.
The independent clinical provider team evaluates each product category separately to confirm there is a valid medical reason for each purchase.
You may see an error, or receive a message, indicating that your survey was not fully completed. This usually means you attested to a qualifying health condition for one product in your order, but did not complete the eligibility check for the other.
For example, if your cart contained Product A and Product B, and these fall under different health categories:
You may have attested to a diagnosis that supports Product A. The survey for Product B was either skipped or not completed.
As a result, the full order cannot be qualified, and your HSA/FSA card cannot be used for the entire purchase.
How to resolve it
- Identify which product’s eligibility check is incomplete. The survey flow or any follow-up communication you received should indicate which product needs attention.
- Return to the survey and edit your answers.
- Attest to a qualifying diagnosis for the remaining product. When prompted about health conditions, make sure you are responding to the questions for each product in your order, not just the first one.
- Complete and submit the full survey. Once you have attested to a qualifying condition for each product category, the survey will complete and your order will be eligible for HSA/FSA payment.
If you genuinely do not have a health condition that aligns with one of the products in your order, that product may not be eligible for HSA/FSA payment. The qualification requires demonstrating a real medical reason, not general wellness intent. You can still complete the purchase; the ineligible product would simply need to be paid for separately with a non-HSA/FSA method.
Truemed requests health information so the clinical provider team can evaluate whether there is a medically necessary reason for your purchase of a specific product or service. Identifying a specific health condition, rather than a general wellness goal, is a requirement under IRS rules for customers who want to use HSA or FSA funds on certain products.
Sometimes the cleanest fix is to begin the survey fresh. Here is what to know before you do.
Check your original order confirmation email. The qualification survey link is typically included there. If the link has expired, it will prompt you to request a new one.
Contact support. If you cannot locate a working link, reach out to support@truemed.com and the team can send a new survey link tied to your order.
Restarting is appropriate when:
- The original survey link has expired and you cannot complete it.
- You started a survey but navigated away before finishing.
- You completed a survey but made errors you want to correct (contact support first to confirm).
- Technical issues prevented the survey from loading or submitting properly.
Many survey issues can be resolved without reaching out, but some situations genuinely require help from the Truemed team.
Reach out to support if:
- Your survey shows as complete on your end, but you have not received a confirmation email or LMN after more than 24 hours.
- You believe you completed all required fields but keep receiving an “incomplete” error.
- You cannot get the survey to load at all, even after trying a different browser.
